You can now pre-register your child(ren) to receive the H1N1 vaccine.  Follow these three simple steps:
1.  Click on the following link.  [ www.FluShotsUSA.com/schools ]  
2.  Select your school district and school.  Enter information as prompted.
3.  Print out the confirmation / consent form for each of your children

Once registered, you will receive an e-mail confirmation when the date has been set.
